6 months Job Summary We are seeking an experienced Hospice Registered Nurse (RN) for a travel contract in Basking Ridge, NJ. The RN will provide comprehensive hospice care in a home setting, manage patient cases, perform assessments, coordinate care, and support patients and families throughout the hospice process. Responsibilities Perform comprehensive patient assessments and determine appropriateness for hospice care. Develop, implement, and evaluate individualized plans of care for patients and families. Manage an assigned hospice caseload and prioritize patient needs. Monitor patient progress toward established goals and revise care plans as needed. Provide patient and family education using age-appropriate teaching methods. Complete admission, discharge, transfer, and revisit documentation within required timeframes. Maintain accurate documentation in the EMR. Coordinate care with the interdisciplinary team and participate in IDT meetings. Communicate patient progress and changes in condition to appropriate members of the care team. Perform medication reconciliation and maintain current medication orders. Follow infection-control practices and maintain a safe environment for patients and coworkers. Provide HHA supervision according to established standards. Demonstrate knowledge of hospice regulations and insurance reimbursement guidelines. Participate in performance improvement activities and staff meetings. Provide orientation and mentorship to new employees as needed. Travel to patient locations and provide care across assigned counties according to operational needs. Required Qualifications Current New Jersey RN License Minimum 2 years of RN experience in a home hospice environment Valid New Jersey Driver's License Current BLS certification Experience using an EMR Strong critical-thinking skills Excellent communication skills Ability to work independently in a home-care/hospice environment Associate's degree in Nursing (ASN) or qualifying nursing diploma with NJ RN license Preferred Qualifications 3+ years of RN experience Additional hospice/home-care experience Apply through Indeed with your current resume and contact information.Pay:
