Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members in their homes Evaluate patients current and previous medical histories, conditions, and medication regimens Offer education to patients on medical issues and proper medication use Identify care gaps, offering recommendations to address any discovered needs Assessment visits may include annual health reviews or post-discharge check-ins No responsibility for creating treatment plans or medication prescribing required

Qualifications:
