Upto $51/hr ???? Key Responsibilities

  • Provide safe, competent, and evidence-based nursing care to patients in their homes
  • Assess patient needs and develop, implement, and evaluate individualized plans of care
  • Collaborate with patients, families, providers, and interdisciplinary care teams
  • Provide nursing interventions including IV therapy, peripheral IV access, lab specimen collection, and wound care
  • Monitor patient progress and ensure care is delivered according to provider orders and the plan of care
  • Educate patients and caregivers on treatment plans, medications, disease prevention, and post-discharge care
  • Manage an assigned caseload and ensure patients progress toward established goals
  • Document patient care accurately in the EMR
  • Identify and escalate safety concerns, barriers to care, and changes in patient condition
  • Participate in quality improvement, evidence-based practice, and patient safety initiatives ?

??? Requirements ? Minimum 2 years of Registered Nurse experience ? Current New York State RN License ? BLS certification from AHA or Red Cross ? Relevant clinical experience and strong nursing assessment skills ? Excellent communication, interpersonal, critical-thinking, and problem-solving skills ? Ability to work independently while collaborating effectively with interdisciplinary teams ? Proficiency with Electronic Medical Records (EMR) and basic data-entry systems ? Ability to perform clinical procedures such as venipuncture and other nursing interventions ?
