Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage plan members Evaluate comprehensive health histories, including current and prior conditions, and review medication usage Provide valuable education to members about their medical diagnoses and treatment regimens Identify care gaps and recommend actionable next steps for improved health outcomes Focus exclusively on evaluation and educationno requirement for prescribing or developing treatment plans

Qualifications:

Valid Nurse Practitioner License in New Jersey Active CPR certification (BLS) Board Certification via AANP or

ANCC MSN
