Home Health RN Employment Type:
Contract-to-Hire (CTH Eligible)
Schedule:
Monday-Friday, 8:00 AM-5:00
PM Location:
Burlington, Camden & Gloucester Counties, NJ Pay Rate:
$45.00-$50.00/hour Pay Scale Based on
Experience Mileage:
Reimbursed at the government rate Position Summary The Home Health RN is responsible for assessing, diagnosing, planning, implementing, and evaluating patient care in the home using evidence-based practice and outcome identification. The RN will coordinate patient care and collaborate extensively with healthcare team members, patients, families, and significant others to develop and implement individualized plans of care. The RN will also supervise Home Health Aides, promote quality care, and support efficient utilization of agency resources. The RN will visit approximately 5-6 patients per day throughout Burlington, Camden, and Gloucester Counties. Required Qualifications Active New Jersey RN license 3-4 years of nursing experience preferred in: Med-Surg
ICU/ICE CCU
Home Health Strong core nursing skills Ability to perform and document manual vital signs Preferred Qualifications OASIS experience - required for candidates coming from certified home health Certified Home Health experience Previous LPN background Hospital nursing experience
