A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

POSITION SUMMARY
Reporting to the Director of Nursing, the Registered Nurse (RN) provides nursing care in accordance with the patient's plan of care, including comprehensive health and psychosocial evaluation, monitoring of the patient's condition, health promotion and prevention, coordination of services, teaching and training activities, and direct nursing care. This position carries a high risk of exposure to bloodborne pathogens.
RESPONSIBILITIES
General Responsibilities Coordinates total patient care through comprehensive health and psychosocial evaluation, condition monitoring, preventive care, service coordination, and patient/family teaching. Evaluates the effectiveness of nursing services provided to the patient and family on an ongoing basis. Completes admission, transfer, recertification, resumption of care, and discharge paperwork. Prepares and presents patient records to the Clinical Record Review Committee as indicated. Consults with the attending physician and appropriate supervisor regarding changes to the patient's care plan. Submits clinical and progress notes and other clinical record forms no less than weekly (within 72 hours as applicable), along with a daily tally of patient visits. Participates in case conferences and discusses patient care needs, including the involvement of other care team members (home health aide, physical therapist, occupational therapist, speech therapist, social worker), with the supervisor. Obtains orders for paraprofessional services and submits referrals to appropriate personnel. Participates in the patient's discharge planning process. Cooperates with other agencies providing nursing or related services to ensure continuity of care. Participates in staff development meetings and student nurse education experiences. Maintains ongoing knowledge of current drug therapy. Adheres to federal, New Jersey state, and accreditation requirements. Participates in the planning, operation, and evaluation of nursing services. May fill in for other nurses as requested by the Director of Nursing. Continually works to improve nursing skills through in-service education, formal education, workshops, conferences, and active participation in professional organizations. Patient Admission and Assessment Conducts an initial and ongoing comprehensive assessment of the patient's needs, including medical history, physical examination, vital signs, mental status, and diet. Evaluates the patient, family, and home environment to determine health teaching needs and the level of family support available. Determines whether Home Health Aide services are required and at what frequency. Explains agency nursing and other services to patients and families as part of care planning. Develops and implements the nursing care plan. Skilled Nursing Care Provides nursing services, treatments, and preventive procedures requiring specialized skill, as ordered by the physician. Initiates preventive and rehabilitative nursing procedures appropriate to the patient's care and safety. Observes and reports to the physician signs, symptoms, and reactions to treatment, as well as changes in the patient's physical or emotional condition. Teaches, supervises, and counsels patients and caregivers regarding nursing care needs. Participates in the development and periodic revision of the physician's Plan of Treatment, processing change orders as needed. Supervision of Home Health Aides and LPNs Supervises and evaluates care provided by Certified Home Health Aides (CHHHAs) and LPNs per agency policy, and submits written evaluations. Prepares the care plan for the CHHHA. Charts services rendered by staff nurses and aides, and any changes in patient condition or home situation, revising the care plan as needed. Participates in periodic conferences with the aide supervisor regarding aide performance. Closes charts of discharged patients.
JOB CONDITIONS
Must have a valid driver's license and be willing and able to drive to patients' residences. Must be able to access patients' homes, which may not be routinely wheelchair accessible; hearing, eyesight, and physical dexterity must be sufficient to perform and demonstrate patient care. Physical activities include walking, sitting, stooping, standing, and minimal to maximum lifting and turning of patients. Must be able to communicate effectively both verbally and in writing, as frequent communication by telephone and in writing in English is required.
QUALIFICATIONS
Graduate of an accredited School of Nursing. Current RN license in the State of New Jersey. Two or more years of experience in a community/home health services setting or hospital setting is preferred. Working knowledge of home care and the principles and techniques of professional nursing, including required documentation. Strong organizational and time-management skills, with knowledge of management processes. Ability to contribute to the quality of care through constructive communication with nursing managers and staff. Successful completion of a criminal background check. Current CPR certification.
COMPENSATION
$45.00 per hour, full-time. Actual hours are subject to change based on client care requirements.
