Responsibilities:

Adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards Open communication with other Team Members, Clients, and Family Provide input regarding placing care partners appropriately with clients Supervision of CHHA team members for on-campus / off-campus clients Making monthly follow-up calls/visits to ensure adherence of Care Plan Evaluate effectiveness of care provided by CHHAs with timely completion of 60-day skills assessments during on-site visit

Requirements:

New Jersey Registered Nurse (RN) License BLS Certification New Jersey Driver s License, and an automobile in good working condition with appropriate insurance coverage in accordance to New Jersey laws. Minimum of one year experience in health care setting, preferably home health, hospital, or long-term care Ability to join on-call rotation

Skills:

Exceptional communication and interpersonal skills Robust critical thinking and problem-solving abilities Capable of working independently and collaboratively in a multidisciplinary team Proficient in electronic medical record systems and relevant software applications
