Experience with Adult Day Medical Care Center, Bi-lingual a must, English/Spanish. Monday - Friday, some Saturday as needed. Full time and Part Time available. Salary based on experience. Responsibilities Regarding Participant Health Care:
1. Provide nursing care based upon physician's orders and daily assessment of each participant's health status; 2. Upon admission and quarterly thereafter develop and review the nursing portion of the participant care plan; 3. Provide observation, monitoring and intervention for unstable medical conditions; 4. Provide liaison with the participant's attending physician and consulting physicians, notifying them of any changes in the health status of the participant; 5. Take day attendance and record daily statistical data as required, 6 Communicate with the family caregiver/participant representative regarding problems related to care of the participant at the center or at home ranging from changes in the participant's condition or physician's orders to concerns regarding the participant's hygiene or dietary/fluid intake, 7. Provide health education and counseling regarding the participant to family / participant representative, 8. Administer, or supervise, as prescribed by the participant's attending physician, the pharmaceutical regime for each participant, document same and observe for adverse reactions; 9. Provide emergency care including notification of ambulance, physician, emergency care facility and participant's emergency contact; 10 Supervise the provision of meals and the participant's dietary and fluid intake, 11 Coordinate the provision of other health services provided within and outside of the center; 12. Supervise in the monitoring of routine care, not limited to vital signs and weight; 13. Supervise in the provision of direction to program aides assisting with Personal care; incontinence care and maintenance of supplies related to same, 14. Supervise the in the provision of prophylactic skin care to avoid breakdown and nail care 15. Train nursing staff and supervise all staff in the use of standard protocols for communicable diseases and infection control (including OSHA regulations); 16. Document all nursing services in the participants medical record 17. Procedures for the notification of the administrator if there are significant changes in a participant's condition. 18. Manage participant medical records including periodic review for content and completeness; Periodically inspect drug storage area, treatment supplies, emergency equipment and personal care supplies.
