Assumes responsibility for case management. Prioritizes schedule to accommodate patient's needs. Evaluates the plan of care and revises as needed. Demonstrates knowledge of pharmacology and safe medication management and end-of-life symptom management. Routinely reconciles medications. Maintain current medication orders. Shows evidence of interdisciplinary communication. Participates in PI meetings and activities/committees as requested. Assists with growing agency business. Provides orientation to new employees. Accepts other assignments as requested. Cross trains to learn new skills and demonstrates a willingness to teach others.
Required:
1. Graduate of an accredited Nursing Program. 2. Current state licensure as a Registered Nurse. 3. BLS certification.
Preferred:
1. Bachelor's degree in nursing. 2. All ASN must complete their BSN studies within five years of their start date. 3. 1+ year of experience in Hospice Nursing 4. CHPN certification
