RN Case Manager - Camden County (Full-Time) A compassionate and dedicated job-seeker is needed for the Hospice Nurse opportunity with our company! As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients. What You'll Do:
Perform patient assessments for comfort and relief of symptoms. Establish and implement an individualized plan of care for each patient. Assess the need for patient and family education and provide education as needed. Obtain appropriate physician orders to achieve symptom management. Triage and arrange home visit schedule, demonstrating efficiency in time and mile management. Make referrals to additional IDT members and collaborate with social workers related to their involvement. What You'll Bring to
Our Team:
Minimum of two years of nursing experience. NJ RN license required; BSN preferred Bilingual is a plus!
