2988263602-2 Apply for this job now Job Description HarmonyCares is seeking a per-visit Family Nurse Practitioner to perform in-home Health Risk Assessments (HRAs) for medically complex, home-limited patients. In this PRN role, you will conduct comprehensive histories and physicals, identify care gaps and risk factors, and collaborate with an interdisciplinary team to optimize care plans. You will document thoroughly in the EMR, support preventive and chronic care management, and help reduce avoidable hospitalizations. Responsibilities Conduct in-home health risk assessments for medically complex and home-limited patients Obtain comprehensive medical histories, perform physical exams, and update care plans Identify gaps in care, preventive needs, and social determinants impacting patient health Document visits accurately in the EMR and communicate findings to the interdisciplinary team Collaborate with physicians, nurses, and care managers to coordinate follow-up services Provide patient and caregiver education on conditions, medications, and self-management Use telehealth tools as appropriate to support ongoing patient monitoring Ensure adherence to clinical guidelines, quality metrics, and regulatory requirements Travel within assigned territory to patient homes on a per-visit basis Uphold Harmony Cares values of compassion, respect, and integrity in all interactions Required Skills Health risk assessment (HRA) administration Primary and family care clinical skills Home-based patient assessment Geriatric and chronic disease management Electronic Medical Record (EMR) documentation Care planning and care coordination Telehealth and remote monitoring tools Patient and caregiver education Regulatory and quality compliance in healthcare Time management and independent field practice
