Registered Nurse Hospice Case Manager Field Position

Compensation:
$53 - $56 hourlyResponsibilities:
Coordinate and manage comprehensive care plans tailored to each patient's unique needs, ensuring a holistic approach to hospice care. Collaborate with our multidisciplinary team to deliver seamless, compassionate support that enhances the quality of life for patients and their families. Conduct regular assessments to monitor patient progress, adjusting care plans as necessary to meet evolving needs. Provide emotional and educational support to patients and families, empowering them with the knowledge and resources needed during this critical time. Ensure timely and accurate documentation of patient care activities, maintaining compliance with healthcare regulations and standards. Facilitate effective communication between patients, families, and healthcare providers to ensure a unified approach to care. Participate in ongoing professional development opportunities to enhance your skills and contribute to our culture of continuous learning and improvement.Qualifications:
Current Registered Nurse (RN) license in the state of New Jersey. Experience in hospice or palliative care, demonstrating a compassionate approach to patient and family support. Ability to develop and manage individualized care plans, ensuring they align with each patient's unique needs. Proven track record of effective collaboration within a multidisciplinary team to deliver holistic care. Strong communication skills to facilitate clear and empathetic interactions with patients, families, and healthcare providers. Proficiency in maintaining accurate and timely documentation, adhering to healthcare regulations and standards. Commitment to ongoing professional development, embracing opportunities to enhance skills and knowledge in hospice care.
