Position Purpose:
- The Registered Nurse (RN) at the PACE Organization is responsible for providing skilled nursing care, prescribing treatments, and developing care plans for participants. This role oversees service delivery and ensures the coordination of care to optimize participant health and well-being.
What You Will Do:
- Provide skilled nursing care, including administering treatments and medications as prescribed.
- Conduct assessments, develop care plans, and collaborate with the interdisciplinary team (IDT) to monitor participant health.
- Educate participants and families on self-care, medication use, and health management.
- Ensure adherence to professional nursing standards and regulatory requirements.
- Serve on committees and participate in the development of Quality Assessment and Process Improvement (QAPI) activities.
- Maintain accurate and timely documentation of participant care.

Minimum Qualifications:
- Graduate of an accredited Nursing program with an active, unencumbered RN license (BSN preferred).
- Minimum of 2 years of experience with the geriatric population.
- CPR certification required.
- Valid driver's license and proof of insurance.
- Strong communication skills and proficiency in Microsoft Office.
- Ability to work independently and as part of a collaborative team.
