A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Purpose The Registered Nurse (RN) conducts initial in-home assessments to determine the level of services needed to safely maintain the client's daily activities and quality of life; and is responsible for planning, coordinating, and evaluating services provided to clients and their families in accordance with standard requirements and organizational policy. The RN assumes responsibility for clinical issues and supervision in the absence of the Director of Nursing. Duties and Responsibilities Conducts initial in-home assessment of newly referred clients to determine eligibility and level of home care services to meet client's healthcare needs. Develops and implements care/service plans in conjunction with the client's family and, as necessary, the client's authorized medical practitioner or case manager. Orients the Home Health Aides (HHA) and Companions to the plan of care/service, ensures understanding of the plan and provides necessary instructions. Instructs and/or demonstrates complex assigned tasks or specialized equipment operation as deemed necessary. Performs and documents HHA supervision within the client's home to determine compliance with the plan of care/service and observe skill competency of the HHA. Provides client and family instruction regarding services on an ongoing basis as needed and prior to discharge. Conducts reassessment of visits on a scheduled or as-needed basis to determine adequacy of the plan of care, client response to care/services, and level of satisfaction. Monitors compliance with appropriate/relevant infection control procedures. Discusses any changes in the client's needs or plan of care with client, family, and HHA/Companion. Prepares written documentation and summaries of the client's progress as needed for continuation of home health services. Participates in case conferences with staff to ensure continuity of care. Become familiar with all resources necessary to provide a safe care environment in the home. (e.g., respiratory, DME, etc.) Documents in-home visits, supervisory notes, and client communications accurately and completely in the client's record. Maintains updated professional knowledge and participates in education programs to ensure accurate assessments and appropriate supervisory guidance. Participates as required in after-hours, on call schedule. Maintains HIPAA compliance. Follows agency policies and procedures for all care, services, documentation and maintenance of records. Submits timely documentation to comply with annual health screening, credentialing, and education requirements. Spearheads CAHC audit to ensure compliance and audit completion with distinction. Instructor for the home health aide training academy. Qualifications Minimum 1 year of professional clinical experience as a registered nurse, home health experience preferred Bachelor's degree in Nursing from an accredited nursing program Valid NJ Driver's License and reliable transportation for patient home visits Strong assessment, clinical judgement, communication and documentation skills Teamwork orientation Proficiency in
