Job Description

Assisting Hands Home Care® is seeking a Registered Nurse (RN) in the Monmouth/Ocean County area. This is an opportunity for someone who has a passion for helping seniors and a proven track record of providing exceptional care.
Job Description:
The RN Care Coordinator will be responsible for overseeing delivery of all companion and personal care services delivered by the agency for assigned clients. This includes an initial Nursing Evaluation visit and establish a Plan of Care and 60 day visits as mandated by New Jersey regulatory standards. As a per diem RN, you will be compensated $45/hour which will be calculated "door to door" so you're compensated for every minute spent on the visit including travel time. Documentation is all electronic and can be done on any mobile device and you will be provided with an iPad for charting on the case. Responsibilities and Duties Primary duties will include: Performing client assessments Establish client care plans Maintain appropriate client and staff documentation/records (best practices) as per agency policy Make home supervisory visits to assess client status and caregiver performance Prepare performance evaluations for caregiver staff as required
Experience and Requirements:
Strong clinical and assessment skills Strong interpersonal and customer service skills Excellent communication, organizational and mentoring skills Must be organized, detail oriented, self-motivated, and able to work independently Ability to handle multiple priorities simultaneously
Pay:
$45.00 per hour
