Full Job Description Case Manager Pay Rate:

$55-$58/hr

Certification:

CCMC preferred Responsibilities Coordinate patients throughout the acute care hospitalization . Manage all aspects of the discharge planning and transition process . Perform utilization management and quality screenings . Handle inpatient case management responsibilities. Coordinate care and services to support appropriate patient outcomes and transitions. Required Qualifications Inpatient Case Management experience is required. RN license required. BSN required. 2 years of acute care experience strongly preferred. Experience in case management and managed care. Strong understanding of discharge planning and transitions of care. Preferred Qualifications CCMC certification. 2+ years of acute-care experience. Experience with utilization management. Experience performing quality screenings. ESJSV1
