Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories. This will include initial client assessments and 60 day assessments. Support patients with chronic conditions including geriatrics, memory care, behavioral health issues, and individuals with developmental disabilities or disabilities requiring specialized nursing interventions Proven experience working with diverse patient populations across acute care, long-term care, assisted living, or home health environments.
