Key Responsibilities:
Conduct comprehensive home health risk assessments for Medicare Advantage members Review patients medical histories, current medical conditions, and medication use Educate patients on chronic conditions, preventive care, and medication management Highlight care gaps and recommend actionable steps for improved health outcomes Focus on assessment and educationno development of treatment plans or medication prescribing is needed
Qualifications:
Active Nurse Practitioner license in New Mexico Current CPR certification (BLS required) Board certified through AANP or ANCC At least 6 months of recent NP experience is preferred Master of Science in Nursing (MSN) from an accredited institution Youll visit patients in various locations within Grant County, such as Roswell, Bayard-Santa Rita, Santa Clara, Tyrone, and Hurley. Each home health risk assessment is allotted approximately 45 minutes, and assignments may include annual wellness visits or post-discharge follow-up assessments. This per diem position provides a highly competitive rate of $80-$90 per completed evaluation, plus additional compensation for mileage and time spent on administrative tasks.
