A skilled and compassionate Home Health Registered Nurse is needed to deliver high-quality patient care in a field-based setting. Schedule:

Monday Friday, 8:00 AM 5:00 PM Expectation of 2532 points per week (approximately 35 patient visits daily) Field-based and requires daily travel within an assigned territory (mileage reimbursement provided)

Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management Monitor patient status, adapt care plans as needed, and promptly communicate changes to physicians and care team Educate patients and families about disease processes, medications, and preventive care measures Accurately document electronically using the Home Care Home Base (HCHB) system Collaborate with physical and occupational therapists, medical social workers, and home health aides to support continuity of care Participate in case conferences, meetings, and ongoing education Ensure all care and documentation meet regulatory and agency standards

Qualifications:

Active RN license (in good standing) in the state Minimum one year of recent home health field experience (required) Proficient with Home Care Home Base (HCHB) and OASIS assessments Outstanding clinical assessment, communication, and organizational skills Able to work independently in a flexible, field-based schedule Valid driver's license, reliable transportation, and current auto insurance BSN and experience managing 2532 points per week (preferred) Knowledge of Medicare and home health regulations (preferred) Benefits include weekly pay, a range of medical/dental/vision insurance options, 401(k) plan, ongoing support, mileage reimbursement, and opportunities for contract renewal.
