Key Responsibilities:

Perform comprehensive home health risk assessments, averaging 45 minutes per visit Review and evaluate patients past and current medical history, medication regimens, and ongoing medical conditions Deliver patient education concerning diagnoses, medications, and self-care strategies Identify and report care gaps, offering actionable recommendations to close them No responsibilities for prescribing medication or implementing treatment plans

Qualifications:

Active Nurse Practitioner License in New Mexico Current BLS/CPR certification National Board Certification as a Nurse Practitioner (AANP or ANCC) MSN degree earned from an accredited nursing school At least 6 months of recent nurse practitioner experience preferred Enjoy compensation between $80 to $90 per completed evaluation, with an additional stipend for travel and administrative timeensuring your expertise and time are well rewarded.
