Take on a rewarding Home Health Registered Nurse position, providing essential care and support to patients directly in their homes. This contract opportunity focuses on delivering high-quality, patient-centered care while working independently in a field-based environment.

Schedule:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00

PM Caseload:

2532 points per week (35 patient visits daily)

Key Responsibilities:

Complete comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management Monitor patient progress, communicate condition changes promptly to the care team, and coordinate care with physicians Educate patients and families on diagnoses, medications, and preventative care strategies Maintain up-to-date documentation using Home Care Home Base (HCHB) Collaborate with therapists, medical social workers, and home health aides for integrated, holistic care Engage in case conferences and professional development opportunities Ensure compliance with all agency and regulatory standards

Qualifications:

Active RN license (in good standing) in the state of practice At least 1 year of recent field-based Home Health RN experience Proficient in Home Care Home Base (HCHB) and OASIS assessment tools Reliable transportation, valid drivers license, and proof of auto insurance required Exceptional assessment, communication, time management, and organizational skills Ability to work independently and manage a flexible schedule

Preferred:
