Key Responsibilities:

Complete thorough annual and post-discharge health risk assessments during each home visit (approximately 45 minutes per evaluation) Assess patients prior and current medical history, medical conditions, and medication usage Deliver clear education to patients on their conditions and medication regimens Detect gaps in care and offer recommendations to improve overall health outcomes No need to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in New Mexico Board certification via AANP or ANCC Current CPR/BLS certification MSN from an accredited institution Minimum of six months Nurse Practitioner experience preferred Enjoy a highly competitive rate of $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
