Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members in varied locations within the county (including Hurley, Bayard-Santa Rita, Santa Clara, and Tyrone) Review and analyze patients' prior and current medical histories, health conditions, and medications Educate patients regarding their medical conditions and prescribed medications Identify care gaps and recommend strategies to address unresolved health needs Complete each assessment within approximately 45 minutes, focusing on evaluation rather than ongoing treatment or prescribing

Qualifications:
