Key Responsibilities:
Perform comprehensive annual home health risk assessments for Medicare Advantage members Collect and evaluate detailed medical histories, conditions, and medication information Provide clear education related to health conditions and prescribed medications Assess for potential care gaps, communicating actionable recommendations for improvement Visits may include annual health assessments or post-discharge evaluations; each typically takes about 45 minutes No treatment planning or prescribing necessary
Qualifications:
Active Nurse Practitioner license in New Mexico Board Certification (AANP or ANCC) MSN degree from an accredited program Current CPR certification (BLS) At least 6 months of recent NP experience preferred
