In this per diem Nurse Practitioner role, you'll provide essential healthcare insights during home visits across Santa Fe, Espaola, Edgewood, and other nearby areasdelivering both annual and post-discharge assessments while helping ensure continuity of care.

Key Responsibilities:

Travel to patients homes to conduct annual health risk assessments and post-discharge visits Assess and document comprehensive medical, medication, and behavioral histories Educate patients on existing medical conditions and current medications Identify gaps in care, providing recommendations and resources as needed No requirement to develop treatment plans or prescribe medications

Qualifications:

Valid Nurse Practitioner License in New Mexico Active Board Certification (AANP or ANCC) MSN from an accredited nursing school Current BLS/CPR certification At least 6 months of Nurse Practitioner experience is preferred Compensation for this opportunity ranges from $80 to $90 per evaluation, with an additional mileage stipend and compensation for administrative time.
