Key Responsibilities:

Perform comprehensive home health risk assessments, generally lasting about 45 minutes per visit Evaluate clients past and present medical histories, current medical conditions, and medication use Educate patients regarding their medical issues and medications Detect and provide recommendations for closing any gaps in care Assignments may include annual health assessments or post-discharge visits No responsibility for developing treatment plans or prescribing medication

Qualifications:

Active Nurse Practitioner License in New Mexico Current BLS CPR certification Board Certification through the AANP or ANCC Minimum of 6 months recent NP experience preferred Master of Science in Nursing (MSN) from an accredited institution As part of this position, youll earn $80-$90 per assessment, plus receive a mileage stipend and compensation for administrative time.
