Key Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage members Review and evaluate prior/current medical history, chronic conditions, and medication adherence Offer patient education regarding disease management and prescribed medications Identify care gaps and recommend follow-up actions to enhance health outcomes Focus exclusively on assessment and educationno prescribing or treatment planning required

Qualifications:

Active Nurse Practitioner license in New Mexico Current Basic Life Support (BLS) CPR certification Board certification via AANP or ANCC Master of Science in Nursing (MSN) from an accredited program At least 6 months of recent nurse practitioner experience preferred Enjoy a competitive compensation structure of $80-$90 per completed evaluation, with additional mileage stipends and payment for administrative time.
