Schedule & Key Details:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Typical caseload: 2532 points per week (generally 35 patient visits daily) Requires daily travel within an assigned territory (mileage reimbursement provided) Clinical documentation through Home Care Home Base (HCHB), using the OASIS assessment tool

Core Responsibilities:

Perform OASIS-based assessments to create and revise individualized care plans. Deliver hands-on skilled nursing interventions including wound management, IV therapy, medication administration, and chronic disease monitoring. Evaluate patient status, promptly communicate changes, and collaborate with interdisciplinary teams to coordinate care. Educate patients and families about disease conditions, medications, treatment plans, and preventive practices. Accurately document all care, visits, and communications in HCHB. Participate in case conferences, team meetings, and applicable continuing education. Uphold regulatory and home health agency standards in all aspects of patient care.

Qualifications:
