Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

The RN will provide nursing oversight, coordinate healthcare services, develop care plans, administer medications, and collaborate with interdisciplinary teams to promote the health, safety, and well-being of the individuals served.
Locations:
Somers, NY and Armonk, NY Pay Rate:
$48
  • $52 per hour (based on experience)
Schedule:
Monday through
Friday, Days Available Shifts:
7:00 AM
  • 3:00 PM, 8:00 AM
  • 4:00 PM, or 9:00 AM
  • 5:00 PM Responsibilities Administer medications and treatments as prescribed.
Complete health assessments and develop individualized care plans. Coordinate care with physicians, families, and residential staff. Monitor health status and ensure accurate documentation. Provide clinical guidance and training to support staff. Maintain compliance with OPWDD and agency regulations. Participate in on-call coverage as needed. Qualifications Active New York State Registered Nurse (RN) license required. New graduates encouraged to apply. Experience with
IDD/OPWDD
populations preferred. Strong communication, organizational, and documentation skills. Ability to work independently and collaboratively within a team environment.
