Mileage reimbursement for personal vehicle use What You Will Do Complete UAS NY assessments during in home visits or via telehealth Review medical documentation and record accurate detailed findings Educate consumers and communicate with providers when needed Maintain timely high quality documentation that meets all NYS standards Manage your schedule route planning and daily workflow Follow all conflict free requirements under the NYS Department of Health Qualifications We Look For What You Need Must have own car for daily travel between assessment locations Public transportation and ridesharing are not permitted 1-2 years of experience as an RN in one-on-one in-home, community, skilled nursing facility, long term and geriatric care required RN license valid in New York BSN is required Familiarity with Uniform Assessment System (UAS)-NY is a plus Strong communication, time management, and Electronic Health Record (EHR) skills Reliable transportation for in-person visits Minimum Qualifications Active RN license At least 22 months of home health care experience within the last 15 years , including one or more of the following: RN/LPN experience with a CHHA, LHCSA, or

MLTCP RN

Care Manager for home- or community-based services (Hospice/Assisted Living included) NP providing primary care to homebound individuals HHA or Personal Assistant experience in certified/licensed home care settings Must meet conflict-free requirements under NYS DOH. Preferred Recent experience completing

OASIS UAS
