Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients current and prior medical history, ongoing conditions, and medication regimen Offer education to patients regarding their medical conditions and prescribed medications Identify gaps in care and deliver recommendations to support optimal health outcomes No prescribing or treatment plan creation requiredfocus remains on holistic assessment and education

Qualifications:

Active Nurse Practitioner license in New York Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree earned from an accredited nursing school At least six months of current Nurse Practitioner experience highly preferred
