WHAT YOU'LL DO

Deliver skilled nursing care in clients' homes Complete assessments and monitor changes in condition Develop and follow individualized care plans Coordinate with clients, families, and the care team Educate and support clients and families Document care accurately and on time

WHAT YOU'LL NEED

Active NY RN license Home care experience preferred (not required) Strong clinical judgment and communication skills Comfortable working independently Valid driver's license and reliable transportation preferred Authorized to work in the U.S. Up-to-date medicals/vaccinations (or willingness to obtain) Completed drug screen or Habituation Form
