Works as part of a community or public health program to coordinate care for patients in between clinical appointments or treatments.
$15,000 Sign on Bonus Admission RN- Oswego Health Home Care The Admission RN's primary function is to include admission assessment and plan of care development. Ensure quality and safe delivery of home health care services. Also, provide direct client care in compliance with nurse practice act, physician orders and plan of care. Participates in multidisciplinary aspects of client care. Documentation meets quality standards, reflects nursing care and interactions, is clear and concise and is completed in a timely manner. Cover two holidays per year. Current NYS nursing license with a minimum of one year recent med/surg, critical care or home care experience.

$36.05-$41.20/hr Employee salary is based on commensurate experience and other qualifications.
