$1000 SIGN-ON BONUS Position Summary The Registered Nurse (RN) supports the health, safety, and well-being of individuals receiving services from Lochland, Inc. through nursing oversight, clinical assessment, medication management, staff education, and healthcare coordination. The RN works collaboratively with the Director of Nursing Services, LPNs, Medical Coordinators, DSPs, House Supervisors, and external healthcare providers to ensure residents receive safe, person-centered healthcare in accordance with applicable federal and NYS laws, OPWDD requirements, and professional nursing standards. Essential Duties and Responsibilities Clinical Nursing Conduct nursing assessments and provide clinical follow-up for residents experiencing changes in health status or medical concerns Initiate, monitor, and discontinue clinical protocols, including Head Injury Protocols, per physician orders and nursing standards Review and implement physician recommendations, discharge instructions, and healthcare orders Monitor residents following hospitalizations, emergency department visits, and other significant healthcare events Assist with the development and review of individualized nursing plans and healthcare protocols Respond to medical emergencies and provide clinical guidance to staff Attend medical appointments and assist with transportation when nursing presence is required Advocate for residents to ensure accessible, appropriate, and person-centered care Medication Management Review and process medication orders and ensure accurate entry into electronic health record and medication administration systems Review medication administration records to identify errors, omissions, and documentation concerns Assess and respond to medication errors per agency policy, physician guidance, and applicable reporting requirements Monitor the effectiveness of healthcare interventions and communicate concerns to the Director of Nursing Services and appropriate providers Maintain accurate, timely, and complete nursing documentation and resident health records Staff Training and Clinical Support Provide nursing education and clinical guidance to LPNs, Medical Coordinators, DSPs, and House Supervisors Train and support staff on medication administration, resident-specific healthcare needs, medical protocols, and signs and symptoms requiring nursing intervention Participate in the training and certification of medication-certified staff per OPWDD requirements and applicable nursing regulations Evaluate staff competency related to delegated nursing tasks and provide additional education or corrective guidance when necessary Healthcare Coordination and Documentation Ensure resident medical records are accurate, complete, current, and maintained per agency requirements Communicate relevant healthcare information to staff, providers, family members, care managers, and other authorized members of the support team Participate in Life Plan meetings and other resident-focused meetings as assigned Assist with transitions of care following hospitalizations, rehabilitation stays, or other changes in healthcare services Conduct Health Risk Screening Tool (HRST) clinical reviews per OPWDD requirements Infection Control Monitor for infectious illness within residential settings and implement appropriate infection control measures Ensure compliance with applicable infection control standards and communicate concerns to the Director of Nursing Services Leadership and Quality Improvement Support the Director of Nursing Services in maintaining safe, effective, and compliant nursing services Participate in the Incident Review Committee and other quality improvement committees as assigned Review incidents, medication errors, and other clinical information to identify trends and improvement opportunities Promote effective communication and collaboration across Nursing Services, Residential Services, Day Services, and external providers On-Call Responsibilities Participate in the paid rotating RN on-call schedule, approximately one week per month Respond to staff calls within 15 minutes to provide clinical triage, guidance, and nursing consultation Respond on-site when clinically necessary and document all on-call communications and interventions per agency procedures Regulatory Compliance Perform all nursing responsibilities within the scope of practice of a Registered Professional Nurse in New York State Maintain compliance with applicable federal and NYS laws, OPWDD requirements, HIPAA, Justice Center regulations, and Lochland policies Immediately report suspected abuse, neglect, mistreatment, or exploitation per the Protection of People with Special Needs Act and agency procedures Maintain all required licenses, certifications, and continuing education Complete all required documentation accurately and within established timeframes Required Qualifications Associate's Degree in Nursing (ADN) and active, unrestricted NYS Registered Nurse license in good standing; BSN preferred Minimum one year of clinical nursing experience; experience supporting individuals with intellectual and developmental disabilities preferred Current CPR and First Aid certification, or ability to obtain within the timeframe established by Lochland Valid driver's license and ability to meet agency driving requirements Ability to participate in the paid rotating RN on-call schedule (approximately one week per month) Strong clinical assessment, critical thinking, and organizational skills Effective verbal and written communication skills Ability to maintain accurate clinical documentation and protect confidential health information Ability to provide education and clinical guidance to staff with varying levels of healthcare knowledge Ability to work independently, manage competing priorities, and respond effectively to urgent situations Ability to collaborate professionally with residents, families, employees, healthcare providers, and care managers Physical Requirements The employee must be able to perform the essential functions of this position with or without reasonable accommodation, including moving throughout residential, administrative, and community settings; performing physical activities associated with nursing assessments; responding to medical emergencies; operating a motor vehicle; and working outside regularly scheduled hours as part of the on-call rotation. Work Environment The RN works in residential homes, administrative offices, healthcare facilities, and community settings. The role involves regular interaction with residents, staff, families, and healthcare providers and may involve exposure to medical conditions, bodily fluids, infectious diseases, challenging behaviors, and emergency situations. $41.00 - $45.00 per hour
