•RN Case Manager

POSITION DUTIES

•Supports safe and timely patient discharge transition (typically 14-18 patients). Coordinates DME and Post acute services. Identifies high-risk patients and discharge barriers, collaborate with interdisciplinary tea members, documents and coordinates care in Epic

MINIMUM REQUIRED QUALIFICATIONS

•NYS RN license NYS PRI Certification Screen Certification BLS 2 years recent case management experience in an acute hospital setting

PREFERRED QUALIFICATIONS

•Experience with Epic and Care Port transition

LENGTH OF ASSIGNMENT

•8 weeks

SHIFT / HOURS PER WEEK

•8:00am-4:00pm, 37.5hrs/week SYSTEMS
