Deliver critical care to patients in their homes by performing comprehensive health risk assessments as a Nurse Practitioner. This per diem opportunity allows you the autonomy to create your own work schedule with flexibility to balance your professional and personal life. You will be assigned visits within Warren County, with each home health risk assessment lasting approximately 45 minutes. Patient visits may include annual health reviews or post-discharge check-ins for Medicare Advantage plan members.

Schedule:

Flexible shifts available Monday through Sunday Self-scheduling to meet minimum weekly or bi-weekly commitments

Qualifications:

Current Nurse Practitioner license in New York Active Board Certification (AANP or ANCC) Current CPR (BLS) certification MSN degree from an accredited program At least 6 months of experience as a nurse practitioner is preferred

Key Responsibilities:

Complete in-home annual and post-discharge health risk assessments for Medicare Advantage plan members Review prior and current medical and medication history Provide education on diagnosis and medication regimens Identify and communicate gaps in care, making recommendations for further follow-up

Note:

This role does not involve prescribing medication or creating treatment plans This position offers compensation of $80$90 per evaluation, in addition to a mileage stipend and administrative time pay.
