Job Description
RN needed to provide case management within group homes serving the developmentally disabled. Monday-Friday - day shift (hours are flexible) 40 hours per week. Responsibilities include updating and maintaining the individuals monthly MARS and health records as required by OPWDD. Schedule and follow up on the individuals medical appointments. Request MD orders or extensions on existing orders or extensions when necessary. Be available to triage individuals if anything should arise while you are in the group home. Sometimes Body checks/Assessments are needed as well as following up with nursing notes. You may also asked to supervise a med pass for AMAP Certifications.
