This position is in The Star Hospice House, our 5 patient hospice residence. Essential Duties and Responsibilities a) Provide personal care as necessitated by the patient need; b) Perform duties per their scope of practice (injections, enemas, medications, etc.); c) Promote expression and venting of feelings and normalization of experience for patient, family and staff; d) Report changes in patient condition to Residence Director, RN Case Manager or After Hours RN; e) Contact Physician for change in orders as patient condition warrants; f) Assist in mentoring Home Health Aides as requested; g) Support and maintain regulatory compliance; h) Maintain timely and appropriate records/documents per Hospice policies and procedures; i) Maintain/enhance professional skills and requirements including education and training; j) Perform other tasks and duties as assigned; k) Take initiative as needed or when the opportunity arises; l) Provide meal preparation and housekeeping as needed; m) Travel as required.
