Up to $56/hour + $90 weekly mileage allowance 1099: Up to $60/hour + $90 weekly mileage allowance

Up to 40 hours

Day Remote Duration:

Up to 40 hours About the Role As an RN Home Health Nurse Assessor , you will play a key role in evaluating patients' health needs and helping develop individualized long-term care plans. This position is ideal for nurses with experience in home health, UAS-NY, OASIS, care management, or comprehensive in-home assessments . ✅ Required Qualifications Active New York RN License . Minimum 2 years of experience working in a HOME setting within the last 15 years . Recent experience conducting comprehensive patient assessments .

Qualifying experience may include:

Certified Home Health Agency ( CHHA ) Licensed Home Care Service Agency ( LHCSA ) Managed Long-Term Care ( MLTC ) OASIS or similar comprehensive assessments Primary care for homebound patients Care management for individuals receiving home and community-based services Hospice or Assisted Living Home care experience as an LPN or Home Health Aide Why Join This Opportunity?
