RN Coordinator - Clinical Services IRI is seeking an RN Coordinator to provide and coordinate quality nursing care for individuals in our residential and day services programs. In this role, you'll oversee clinical care for a supervisory cluster of individuals, working closely with the Director of Clinical Services to implement procedures, ensure regulatory compliance, and drive strong health outcomes across the team.

What You'll Do Clinical Care & Coordination Deliver and oversee quality health care for individuals in IRI's residential and day services programs Provide in-person and phone triage, ensuring the right level of care at the right time Evaluate and manage each individual's plan of care, tracking and documenting health findings Supervise the development of individualized nursing care plans within your cluster Medication Management Administer or supervise administration of prescribed medications, evaluating effectiveness and addressing any concerns Supervise AMAP staff during medication dispensing Coordinate with pharmacies on medication inventory and delivery Train, certify, and re-certify staff in medication administration, infection control, and related topics Compliance & Documentation Ensure compliance with OPWDD, DOH, and CCO regulations, aiming for positive outcomes on all surveys Develop and implement health care policies and procedures (risk management, care plans, triaging, infection control, etc.), subject to Director approval Maintain accurate, standardized medical records using EHR systems Collaboration & Communication Communicate with physicians, specialists, and other providers to ensure individuals receive accurate, coordinated care Work with direct support staff, RNs, and LPNs to ensure consistent, effective care delivery Integrate medical care with nutrition, psychology, speech, OT/PT, and other allied disciplines Participate in clinical meetings, life plan meetings, interdisciplinary team meetings, and emergency team meetings as needed Oversight & Training Ensure timely medical appointments and proper follow-up care Be present for intake and discharge meetings during care transitions Provide technical assistance to residence managers, coordinators, and direct care staff on health and nursing matters Deliver ongoing training on illness signs/symptoms, infection control, CPR, diabetes and seizure management, and more Support clinical data collection and quality assurance initiatives Participate in IRI's nursing on-call schedule About You Bachelor's degree (required) 5+ years of nursing experience with individuals with intellectual/developmental disabilities, including 2+ years in a supervisory role At least 1 year of OPWDD experience Valid driver's license and reliable car Strong interpersonal, communication, and organizational skills Comfort with technology and a detail-oriented approach A team-first, high-energy mindset What We Offer Medical, dental, prescription, and vision coverage — premiums substantially covered by IRI for you, your family, and domestic partners
