Job Summary We are seeking an experienced Registered Nurse (RN) - Case Manager to coordinate patient care, discharge planning, and utilization management in an acute care setting. The RN Case Manager will collaborate with interdisciplinary teams to ensure safe, timely, and effective patient care while facilitating appropriate discharge plans and maintaining continuity of care. Responsibilities Perform initial and concurrent insurance/utilization reviews. Complete psychosocial assessments for patients and families. Develop, implement, and coordinate individualized discharge and treatment plans. Collaborate with physicians, nursing staff, patients, families, and community resources to ensure continuity of care. Complete PRI assessments for patients requiring skilled nursing facility placement. Participate in interdisciplinary rounds, patient conferences, and quality improvement initiatives. Advocate for patients and families while ensuring ethical and legal standards are maintained. Identify and address psychosocial barriers impacting patient care and discharge. Maintain accurate documentation using the EPIC electronic medical record system. Ensure compliance with hospital policies, confidentiality standards, and regulatory requirements. Requirements Active New York State RN License (Required) Minimum 2 years of RN Case Management experience in an acute care setting BLS Certification (Required) Experience with EPIC charting system preferred SCREEN Certification preferred Strong communication, critical thinking, and interdisciplinary collaboration skills RN01
