A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.
$120,000-$135,000 We're partnering with a respected acute care hospital in the Hudson Valley region to find a Registered Nurse Care Manager who's ready to bring their bedside expertise into a care coordination leadership role. As an RN Care Manager, you'll own the full patient journey — from the moment they're admitted through a safe, well-coordinated discharge. You'll work hand-in-hand with physicians, social workers, and families to build care plans that actually fit each patient's needs, not a one-size-fits-all template.
Schedule:
Monday-Friday, 8:30 AM-5:00 PM, with rotating weekends/holidays (40 hrs/pay period)
Responsibilties:
Advocating for patients by negotiating with insurance payers and resolving care-related concerns Assessing patients to determine the right level of care, likely length of stay, and discharge readiness Building physician-directed, individualized post-hospital care plans in partnership with the interdisciplinary team Applying clinical pathways to guide outcomes and flagging variances that need extra attention Coordinating home care services, medical equipment, and transfers to rehab, skilled nursing, or other facilities Keeping documentation (PRI/Screen, HC-485, Level 1, etc.) accurate and timely Contributing to performance improvement projects that raise the bar for patient care Staying sharp on regulatory and payer guidelines, including Joint Commission and NYSDOH standards Communicating with warmth and professionalism across every patient, family, and colleague interaction Continuing to grow through ongoing training and educational opportunities
Requirements:
Active Registered Nurse license in New York State Associate Degree in Nursing required; Bachelor's Degree preferred 4-6 years of nursing experience, ideally in Med/Surg, ICU, or ER Sharp assessment and critical-thinking skills paired with genuine empathy Confidence handling conflict resolution and difficult conversations
