$101,000 - $117,000 per year Job Details - RN Case Manager seeking on average only 4 patients per day within facilities a homes. Roughly 75% facilities. What You'll Be Doing Manage a caseload of hospice patients throughout Midtown Manhattan, completing routine visits in both skilled nursing facilities and patient homes (approximately 75% facility-based care). Perform comprehensive assessments, develop individualized care plans, and provide symptom management focused on comfort and quality of life. Educate patients, families, and caregivers regarding disease progression, medications, hospice services, and end-of-life care. Collaborate with physicians, social workers, chaplains, and the interdisciplinary care team to coordinate and deliver exceptional hospice care. Skills You Need Active RN license with experience in Hospice, Palliative Care, Oncology, Hospital, Rehabilitation, Skilled Nursing, Home Health, or another setting where you've worked directly with hospice or end-of-life patients. Strong clinical assessment, care coordination, communication, and patient/family education skills with a compassionate, relationship-focused approach to care.
