00 per year Description SUMMARY As a Community Health Nurse, you will focus on providing nursing interventions, patient education, and care management to a variety of clients in our community. This promotes good health and one on one patient care in the comfort of their own home.

Schedule:

Varies, Weekends required and one other day during the work week, 30 hours, get paid for 40 RESPONSIBILITIES Patient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care Planning. Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly; coordinates client services and/or referrals in an efficient and timely manner Documentation. Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards Compliance. REQUIRED QUALIFICATIONS

Diploma or Associate's Degree in Nursing Registered Nurse license in New York State. Valid NYS Driver's License.

PREFERRED QUALIFICATIONS

Bachelor's Degree in Nursing preferred. 1 year of nursing experienced preferred. Ability to work independently. Prior home health, clinical and direct patient care experience preferred.

These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision-making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on-call schedules are typically required.
