Schedule:

Monday Friday, 8:00 AM 5:00 PM Field-based role with local travel (mileage reimbursement provided) Manage a caseload of approximately 30 points per week (35 patient visits daily)

Key Responsibilities:

Perform thorough OASIS assessments and develop personalized, goal-oriented care plans Deliver skilled nursing interventions, including medication administration, wound and IV care, and chronic disease management Monitor patient progress, identify any changes in condition, and communicate rapidly with the interdisciplinary care team and physicians Provide comprehensive education to patients and families regarding disease processes, medication regimens, and safe care in the home Accurately document all patient care using Home Care Home Base (HCHB) Collaborate closely with other health professionalsPTs, OTs, social workers, and aidesfor fully coordinated, quality care Participate in team conferences, staff meetings, and professional development activities Maintain compliance with all regulatory and agency standards

Qualifications:

Current RN license in good standing in the state of practice At least 1 year of recent home health nursing experience (field preferred) Skilled in using Home Care Home Base (HCHB) and completing OASIS assessments Reliable transportation, valid drivers license, and active auto insurance
