Nurse Coordinator Shepherd Home - 4.4 Penfield, NY Job Details Part-time | Full-time | Contract $27 - $28 an hour 2 hours ago Benefits Paid time off Qualifications RN License Problem-solving End-of-life care Organizational skills Hospital experience Time management Full Job Description Overview Shepherd Home is a two-bedroom home in Penfield,NY that operates under the hospice philosophy, providing 24-hour care for the terminally ill in a residential setting. Shepherd Home offers physical, emotional and spiritual support to those facing the prospect of death. We also provide hospitality and support to family members and friends as well as bereavement counseling and community education. Duties Coordination of resident care with the Executive Director Meet with families and social workers to access and evaluate resident home hospice plan of care Work with hospital social workers and medical teams to ensure resident safety and comfort Support residents and families served by Shepherd Home Responsible for supervision and scheduling of overnight CNA's, assists with resident assessments Responsible for coordinating and scheduling weekend and on-call nursing teams Responsible for conducting and implementing volunteer orientation and in-service quarterly training Experience NYS license LPN with experience in Hospice care, RN preferred 3-5 year's of related experience required in community health/hospice, medical/surgical or skilled nursing facility. Strong communication, decision-making and problem solving skills 2+ years of supervisory experience, preferred Time management and organizational skills Proven ability to work in a team environment Experience in Microsoft Office Join us in delivering safe, compassionate care to individuals and families facing the end-of-life journey.
Pay:
$27.00 - $28.00 per hour
