Day/Evening (D/E) Requirements Valid NY RN license Current BLS certification Proven experience working in long-term care facilities or nursing homes with familiarity in geriatrics and dementia/dementia care. Responsibilities Deliver comprehensive nursing care to residents in a long-term care setting, including medication administration, wound care, and vital signs monitoring Conduct thorough patient assessments, including physical examinations and health evaluations, to develop tailored care plans Manage complex medical procedures such as IV infusions, catheterizations, ostomy care, and tube feeding with precision and aseptic technique Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and specialized therapies such as memory care or hospice & palliative medicine Maintain accurate medical documentation within Electronic Health Records (EHR) systems like Cerner or EMR/EHR platforms, ensuring compliance with HIPAA regulations Respond promptly to acute pain management needs, triage urgent cases, and provide critical care when necessary Educate residents and their families on health coaching, medication administration, and disease management strategies #RNR #nurses2026
