Key Responsibilities:

Carry out comprehensive home health risk assessments for Medicare Advantage participants Review medical history, evaluate ongoing medical conditions, and assess medication usage Deliver clear education to members regarding their health conditions and prescribed medications Detect potential care gaps and collaborate with care management for effective follow-up solutions No responsibility for developing treatment plans or prescribing medications

Desired Qualifications:
