Job Description

Schenectady Center seeks a compassionate Registered Nurse to provide skilled nursing care to short-term rehab and long-term care residents in our Schenectady, NY facility. The RN will complete assessments, administer medications and treatments, monitor changes in condition, and collaborate with the interdisciplinary team to implement individualized care plans. Responsibilities include supervising CNAs, documenting in the EMR, educating residents and families, and supporting safe transitions of care. Responsibilities Provide direct nursing care to short-term rehab and long-term care residents Complete comprehensive nursing assessments and ongoing monitoring Administer medications, treatments, and therapies safely and accurately Develop, implement, and update individualized care plans with the care team Supervise and support CNAs and other support staff during shifts Document assessments, interventions, and outcomes in the EMR accurately and timely Communicate changes in resident condition to providers and families Educate residents and families on care plans, medications, and health management Participate in interdisciplinary team meetings and quality improvement initiatives Maintain infection control, safety, and regulatory compliance standards Required Skills Patient assessment Medication administration Care planning Wound care IV therapy Electronic Medical Records (EMR) documentation Infection control Patient and family education Clinical supervision of support staff Regulatory and compliance knowledge (long-term care)
