Job Description

Key Responsibilities:
Assess, monitor, and evaluate the health status of people receiving services. Develop, implement, and update individualized nursing care plans (PONS and related documents). Review medical records, lab results, diagnostic tests, and treatment plans; coordinate follow-up care as needed. Provide health education to individuals, families, and staff regarding medications, treatments, diets, and wellness practices. Monitor and document changes in health status, responses to treatments, and clinical concerns. Triage health-related calls and provide clinical recommendations, including after-hours support as required. Maintain accurate and timely clinical documentation in accordance with regulatory and agency standards. Collaborate with physicians, hospitals, and other healthcare providers to coordinate care, including during hospitalizations. Train and support direct care staff on medication administration, medical protocols, infection control, and specialized healthcare needs. Participate in intake assessments, interdisciplinary meetings, quality assurance activities, and incident review processes. Provide oversight and guidance to LPNs and support the development of nursing staff when applicable. Participate in an on-call rotation to ensure nursing coverage and emergency response.
Minimum Qualifications:
Active Registered Nurse (RN) license in New York and/or New Jersey. Current Basic Life Support (BLS) certification or ability to obtain within 90 days of hire. Strong clinical assessment skills and sound nursing judgment. Excellent communication and interpersonal skills, with the ability to effectively educate and collaborate with individuals served, staff, families, and healthcare providers. Experience training and supporting staff on medication administration, healthcare needs, and specialized medical care. Proficient with electronic medical records (EMR/EHR), Microsoft Office, email, and related healthcare software systems. Highly organized with the ability to manage multiple priorities and meet deadlines. Willingness to obtain and maintain additional certifications as required (e.g., SCIP-R, Diabetes Care, Train-the-Trainer). Ability to travel between assigned programs and provide on-site nursing support as needed. Ability to participate in on-call rotations and respond to medical concerns or emergencies outside regular business hours. Ability to meet the physical demands of the role, including standing, walking, lifting up to 25 pounds, assisting with transfers, and supporting individuals during emergency interventions when necessary.
Preferred Qualifications:
Previous nursing experience in any setting (hospital, clinic, etc.). Two years of experience supporting people with I/DD, mental health or related needs and/or individuals in crisis in any capacity including in a nursing home, assisted living facility, rehabilitation center and/or setting applicable to assigned caseload. Working knowledge of I/DD, related diagnoses and applicable regulations in state of assignment (OPWDD, DDD, etc.).
Schedule:

$85,000.00 - $95,000.00 per year
